We have contracted with a variety of insurance providers to accommodate the needs of our patients. We are currently an in-network provider for the insurance carriers to the right.
As a courtesy, we will bill your insurance on your behalf. Please check with your individual health plan to ensure that the services you are requesting are covered under your benefit plan. If your plan does not cover the services provided, you may be billed for these services.
Insurance companies dictate that all Co-Pays are due at the time of service.
Please come prepared to submit your co-pay at the time of your appointment.
After your insurance carrier processes your claim, we will bill you for the portion of the balance that they dictate as "patient responsibility" (ex. Co-insurance, deductibles, non-covered services).

Self-Pay
The Family Doctors offers a ‘time of service’ discount for anyone paying out of pocket.
